Whether you are booking a first appointment or just working out whether we are the
right fit, this is the place to start. The form further down goes straight to reception.
If this is an emergency, do not use this form.
Call 911. For a mental health or suicide crisis, the 988 Suicide & Crisis Lifeline is
free, confidential and answered 24 hours a day by calling or texting 988. Messages sent
here are read during office hours only.
Which Way to Reach Us
- New patient? Use the form below, or call
(770) 674-6311 if you would rather talk it through.
- Already a patient? Please call, or send a message through the
patient
portal, which keeps your message with your record.
- Booking a Mental Health First Aid course? That has its own
signup form.
Worth Having Ready
None of this is required to get in touch, but having it to hand makes the first
conversation a great deal more useful:
- What is actually bothering you, and roughly when it started
- Recent lab work, particularly anything that came back "normal" while you still
felt unwell
- Current medications and supplements, including doses
- Which other providers are involved in your care, since we work alongside them
rather than in place of them
Before You Book
Two pages answer most of what people ask at this stage. Our
new patient guide covers what a first appointment
involves and why it runs longer than you are used to, and the
fees and insurance page sets out what visits cost and the exact
questions to ask your carrier. Our FAQ handles the rest.
